Once you refer a case to Advanced Elderly Services we follow it from acceptance to discharge. Our services depend on referral sources, it is the life force of our existence as a home care/elder care provider. Advanced Elderly Services will work with all referral sources including Hospital Discharge Planners, Family Members, Rehab Facilities, Insurance Companies, Case Managers and Social Workers.
